Coverage R

BLANKET COVERAGE

Provides 100% of the usual and customary charges for medical, dental and hospital services, with no deductible. Drugs, supplies and orthopedic appliances included.

 

Coverage P

BLANKET or INDIVIDUAL COVERAGE

Provides scheduled indemnities for medical, dental and hospital services. Automatic catastrophe allocation at 80% after a $100.00 deductible.

 

Option A

All school sponsored activities: including interscholastic football, on an individual basis.

Option B

All school sponsored activities excluding interscholastic football.

Option I

All school sponsored activities excluding all interscholastic athletics.

  • Athletics included at no additional premium.
  • Catastrophic benefits, $50,000 aggregate max, 3 year benefit period.
  • Accidental death & dismemberment benefits.
  • Deferred dental max $1000, benefits continue to 90 days after HS graduation.
